This issue highlights landmark studies and updated guidelines on
the metabolic syndrome in 2007.

Landmark Studies in 2007 that Focused on Hypoglycaemic Agents

Rosiglitazone Atherosclerosis Study

Outcome:
Change in carotid intima-media thickness.

Number of Participants:
555 subjects with type 2 diabetes mellitus or the insulin resistance syndrome; 447 completed the study.

Comparison:
Rosiglitazone (4 mg/d for 2 months and then 8 mg/d) vs placebo.

Design:
Randomized, double-blind, placebo-controlled.

Landmark Studies in 2007 that Focused on Lipid Modifying Agents

Investigation of Lipid Level Management Using Coronary Ultrasound To Assess Reduction of Atherosclerosis by CETP Inhibition and HDL Elevation (ILLUSTRATE) Study

Outcome:
Change in coronary artery atheroma volume as measured by intravascular ultrasound (IVUS).

Number of Participants:
1188 with angiographic evidence of coronary atherosclerosis.

Comparison:
Torcetrapib/atorvastatis vs atorvastatin.

Design:
Randomized, double-blind, active-controlled, parallel assignment.

Measuring Effects on Intima-Media Thickness: An Evaluation of Rosuvastatin (METEOR) Study

Outcome:
Change in carotid intima-media thickness (assessed with B-mode ultrasound.)

Number of Participants:
981 asymptomatic subjects with moderately elevated cholesterol and low risk of cardiovascular disease.

Comparison:
Rosuvastatin 40mg/d vs placebo.

Design:
Randomized, double-blind, placebo-controlled.

Landmark Studies in 2007 that Focused on Antihypertensive Agents

Jikei Heart Study

Outcome:
Composite of cardiovascular morbidity and mortality.

Number of Participants:
3081 patients who were undergoing conventional treatment for hypertension, coronary heart disease, heart failure, or a combination of these disorders.

Comparison:
(in addition to treatment) Valsartan vs other treatment without angiotensin receptor blockers.

Design:
Randomized, open label, active control, parallel assignment.

Other Landmark Studies in 2007

(EVEREST) Study

Outcome:
All-cause mortality and composite of cardiovascular death or hospitalization for heart failure.

Number of Participants:
4133 patients hospitalized (> 48 hours earlier) for exacerbation of chronic heart failure, with left ventricular ejection fraction < 40 signs of volume expansion, and New York Heart Association class III/IV symptoms.

Comparison:
Tolvaptan vs placebo.

Design:
Randomized, double-blind, placebo-controlled.

Mission of the Metabolic
Syndrome Institute

Created in 2003, the Metabolic Syndrome Institute is an independent and not for profit association. Its members are international experts in lipid metabolism, diabetes, heart disease, endocrinology obesity, genetics, epidemiology, basic research and health economics. Being the first association totally devoted to the dissemination of knowledge about the metabolic syndrome, the Metabolic Syndrome Institute will provide an international multidisciplinary approach to a worldwide public health problem.
